Blood in the urine (hematuria) may be divided into visible (macroscopic or frank hematuria) or nonvisible (microscopic or using dipstick urinalysis) and is an indication for cystourethroscopy to diagnose an underlying condition, such as bladder cancer. However, if there is only a trace of blood on dipstick, it may be discounted without undertaking endoscopy [3]. The past medical history should be reviewed; for example, a woman with new-onset storage urinary symptoms, leukocyturia, and a past history of a difficult placement of a midurethral tape may need cystoscopy to exclude tape exposure. The passage of air (pneumaturia) or feces (fecaluria) in the urine strongly suggests a colovesical fistula, and cystoscopy may reveal inflamed urothelium around the site of the fistula. Continuous incontinence may result from a vesicovaginal fistula or ectopic ureter bypassing the urinary sphincter. The cystoscope should be assembled correctly and connected to sterile irrigant fluid at approximately 90 cm above the patient’s pubic symphysis. Lubrication of the urethral meatus with an anesthetic gel is followed by gentle passage of the cystoscope, following the lumen of the urethra. A thorough and systematic inspection of the urethra and bladder is always required. Approximately 50% of permanent pacemakers implanted at our own and other institutions are for the specific treatment of bradyarrhythmias, caused by sinus node dysfunction. This number is increasing as the number of elderly people in our population rises. This has been associated with an increase in symptomatic disorders of sinus node dysfunction, particularly the bradycardia–tachycardia syndrome. Clinical disorders of sinus node dysfunction can be characterized as abnormalities of automaticity or conduction, or both. Automaticity refers to the ability of pacemaker cells within the sinus node to undergo spontaneous depolarization and generate impulses at a rate faster than other latent cardiac pacemakers. Once generated, the impulses must be conducted through and out of the sinus node to depolarize the atrium. Delay or failure of conduction can occur within the sinus node itself or at the sinoatrial junction in the perinodal tissue. The assessment of the influence of autonomic tone on these parameters can provide additional information concerning inherent sinus node function. Electrocardiographic Features of Sinus Node Dysfunction Because no method is currently available to directly record sinus node activity from the body surface in humans, noninvasive evaluation of sinus node automaticity and conduction must be made by indirect methods. Therefore, we attempt to assess sinus function electrographically by analyzing the frequency and pattern of atrial depolarization, that is, P-wave morphology, frequency, and regularity. Sinus Bradycardia When persistent and unexplained, sinus bradycardia (a rate less than 60 beats per minute [bpm]) is said to reflect impaired sinus automaticity. The value of 60 bpm, an arbitrary one, is extremely nonspecific, and it has led to the misclassification of many normal persons as abnormal. A study that involved 24-hour Holter monitoring of 50 healthy medical students revealed that all the students had sinus bradycardia at some time during the 24-hour period and 26% of the students had significant sinus bradycardia (a rate less than 40 2 bpm) during the day. Because autonomic tone plays such an important role in determining the sinus rate, we believe that an isolated heart rate of less than 60 bpm should not be considered abnormal, particularly in asymptomatic people, unless it is persistent, inappropriate for physiologic circumstances, and cannot be explained by other factors. Sinus bradycardia usually results in dizziness, fatigue, mental status changes, and dyspnea on exertion if chronotropic insufficiency is marked. Sinoatrial Block and Sinus Arrest Both sinus arrest and exit block are definite manifestations of sinus node dysfunction. Although sinoatrial block is a conduction disturbance, it remained unsettled whether sinus arrest actually reflects impaired or absent sinus automaticity or varying degrees of sinus exit block. Recently, use of direct recordings of the sinus node has allowed one to ascertain the cause of such pauses (see later section in this chapter entitled Sinus Node Electrogram). Bradycardia–tachycardia Syndrome In our own experience and the experience of others, the bradycardia–tachycardia syndrome is the most frequently encountered form of symptomatic sinus node dysfunction, and it is associated with the highest 3 incidence of syncope. The syncope is generally associated with the marked pauses following the cessation of paroxysmal supraventricular tachyarrhythmias that occur in the setting of sinus bradycardia (primarily atrial fibrillation). Of importance is the recognition that a prolonged asystolic period occurring in the setting of any form of sinus node dysfunction also implies impaired function of lower (nonsinus) pacemakers. The drugs used to prevent atrial fibrillation or control its rate are often responsible P. When these patients are symptomatic, it is usually fatigue or dyspnea on exertion. Autonomic reflex abnormalities consistent with neurocardiac syncope (see below) are usually 4 present when syncope occurs in patients with isolated sinus bradycardia. Unfortunately, most episodes of syncope or dizziness are paroxysmal and unpredictable, and even 24-hour monitoring may fail to include a symptomatic episode. The use of event recorders has improved our ability to correlate symptoms with sinus node dysfunction. In cases with infrequent episodes of syncope, an implantable event recorder is now available. It must be interrogated as a pacemaker currently, but in the near future, it will have automatic detection. Although asymptomatic sinus bradycardia may be noted frequently, its significance remains uncertain. The appropriateness of the sinus rate relative to the physiologic circumstances under which it occurs is critical in deciding whether there is an abnormality of sinus automaticity. Most pauses were asymptomatic, whereas in the remainder, pauses could have produced symptoms. Pacemakers did not benefit asymptomatic people and failed to prevent dizziness, presyncope, and syncope in one-third of the patients in whom such symptoms were felt to be secondary to the pauses. Other studies have demonstrated pauses >2 seconds 6 7 8 in 11% normals and in one-third of trained athletes.

The current accuracy of modern uroflowmeters is around ±2%–5%, despite the fact that a variety of different physical principles are currently used. Time to maximum flow (second): This is the elapsed time from the onset of urine flow to maximum 835 urine flow. Easy to clean There have been many methods used for urine flow measurement, from measuring the time to void a given volume through audiometric and radioisotopic methods to even include high-speed cinematography. The most common method has been that of Drake [6] modified by von Garrelts and Strandell [11]—the measurement of urine weight. In addition, flowmeters have been produced that use the principles of air displacement, differential resistance to gas flow, electromagnetism, photoelectricity, electrical capacitance, and a rotating disc. Flowmeters employing the principles of weight transduction, a rotating disc, and a capacitance transducer are the best known and the most completely tested and validated of the flowmeters available. The rotating disc flowmeter depends on a servometer maintaining the rotation of the disc at a constant speed. Urine hits the disc, and the extra power required to maintain the speed is electronically converted into a measurement of flow rate. The transducer is in the form of a dipstick made of plastic and coated with metal, which dips into the vessel containing the voided urine. For clinical purposes, the measured and indicated flow rate should be accurate to within ±5% more than the clinically significant flow rate range [12]. The capacitance (dipstick) flowmeter is the least expensive to buy and has the advantage of no moving parts, which means mechanical breakdowns are eliminated. Automatic start and stop facilities in some modern flowmeters assist by minimizing patient and staff involvement during the uroflowmetry (Figure 53. It is essential in the clinical situation that every effort is made to make the patient feel comfortable and relaxed. If these requirements are ignored, psychological factors are introduced and a higher proportion of patients will fail to void in a representative way. Ideally, all free uroflowmetry studies should be performed in a completely private uroflowmetry room/toilet, lockable from the inside, and out of hearing range of other staff and patients. As crouching over a toilet seat causes a 21% reduction in the average urine flow rate [13], patients should be encouraged to sit to void. When video studies are combined with pressure–flow recordings in a radiology department, up to 30% of women may fail to void. Patients should be encouraged to attend for uroflowmetry with their bladder comfortably full. It is desirable that the measured urine flow rates should be for a voided volume within the patient’s normal range. This range can be determined if, in the week before the flow study, the patient completes a frequency–volume chart (urinary diary). On this chart, the patient enters the volumes of fluid consumed and the volumes of urine voided. Recent nomograms, however, provide normal reference ranges for urinary flow rates over a wide range of voided volumes. Abnormal or unusual flow curves and urinary flow rates, however, merit repeating the study. The clinical usefulness of flow rates had been attenuated by the lack of absolute values defining normal limits [14]. As urinary flow rates are known to have a strong dependence on voided volume [6,15], these normal limits need to be over a wide range of voided volumes, ideally in the form of nomograms. Studies on normal values for urinary flow rates in women include those of Peter and Drake [16], Scott and McIhlaney [17], Backman [18], Susset et al. Data and/or statistical analysis in these studies has not allowed effective nomogram construction. Study restrictions have included small patient numbers [19–23]; the use of outmoded or less well-evaluated equipment [15–16] and the incompleteness of data at lower voided volumes [15,18] due in part to the inaccuracy of some equipment at lower voided volumes [15]. Each woman voided once in a completely private environment over a calibrated rotating disc-type uroflowmeter; 46 voided on a second occasion. The maximum and average flow rates of the first voids were compared with the respective voided volumes. By using statistical transformations of both voided volumes and urine flow rates, relationships between the two variables were obtained. This allowed the construction of nomograms, which, for ease of interpretation, have been displayed in centile form (Figure 53. The results, after elimination of “abnormal” data, were much slower urine flow rates overall than those in the Liverpool nomograms and an age dependency of urine flow rates, not normally noted in asymptomatic women [15,22,24,26]. Most commonly, a minimum rate of 15 mL per second is quoted for the same parameter if at least 150 mL (or sometimes 200 mL) has been voided. The practice of artificially imposing minimum limits for the voided volume is difficult to justify [27] and very often impractical. Women with certain states of lower urinary tract dysfunction, those in whom the flow rate might be most important, may not be able to hold 200 mL.
